You are invited to join us for Sunday morning service. We are so excited to worship with you in person!

We can accommodate 50 people, so head over or click the link to Eventbrite, to register!

We will have hand sanitizer plenteous for you, distance between seating groupings, and will minimize physical contact!

Please read and agree to the following liability waiver before you show up at the building. If you are in agreement, please print off, sign, and bring with you to the Sunday Service. The signed waiver will be collected and kept on file. We only need one per household.

(If you do not bring a signed waiver for your household, you will be asked to sign this waiver when you arrive and before you are admitted to the building.)

Liability Waiver

It is my intention and choice to participate in services of Family Life Christian Center (Church).  Church is a tenant at 25W560 Geneva Rd. Carol Stream, IL (Building). 

In doing so,

• I acknowledge the current ‘global pandemic of COVID-19’. I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. 

• I further acknowledge that Family Life Christian Center and Building Management have put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

• I further acknowledge that Family Life Christian Center and Building Management cannot guarantee that I will not become infected with the Coronavirus/Covid-19 while attending Church services.

• I acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending church services.

I attest that:

• To the best of my knowledge I am not infected with the Coronavirus.

• I am not experiencing any symptoms of illness such as cough, shortness of breath, or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

• I have not traveled internationally within the last 14 days.

• I have not traveled to or from a highly impacted area within the United States of America in the last 14 days.

• To the best of my knowledge, I have not been recently exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. 

• I have not been diagnosed with Coronavirus/Covid-19.

I hereby release and agree to hold Family Life Christian Center and the Building harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the Church or the Building, or that may otherwise arise in any way in connection with any services received from Family Life Christian Center or my presence at the Building.

I understand that this release discharges Family Life Christian Center and the Building from any liability or claim that I, my heirs, or any personal representatives may have against the Church or Building with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Family Life Christian Center or the Building. This liability waiver and release extends to the Church and Building together with all owners, partners, and employees.

FAQ’s

We want to make sure you feel comfortable attending service with us! If you do not feel comfortable, we encourage you to still join us online!

Do I need to wear a mask?
Masks are not mandatory. We do ask that if you are experiencing any COVID symptoms or if you have been in contact with anyone who has had symptoms, that you please stay home.

Will there be coffee or bagels?
For now we will not be serving any refreshments.

Will there be bulletins?
To minimize the physical contact, we will not be providing bulletins. All updated church info and news will be on the Family Life Connect app and given verbally during service!

Will there be children’s ministry?
This is still to be determined. If you are a family with children who would like to attend, please email our offices admin@familylife.cc so that we can properly accommodate you!
